We provide SUPPORT & SPECIALIST SUPPORT COORDINATION by a team of Social Workers who understand disability.

FUNCTIONAL CAPACITY ASSESSMENT
"Social Workers can design, implement and assess the effectiveness of therapy supports, which may include assessing and evaluating an individual's functional capacity." NDIS, November 2024.

​Social Workers 100% are able to provide Functional Capacity Assessments.

SOCIAL WORK THERAPEUTIC SUPPORT
Social Workers partner with people to address personal difficulties and structural barriers in their lives. We work with individuals, families, groups and communities.

SCHOOL SUPPORT & SCHOOL TRANSITION REPORTS
Do you have a child who is not receiving adequate support at school? Do you feel that you are not being heard?
Our role includes:

Liaising with the school & all Allied Health

Consultation regarding supports

Review of Learning Plans

School Transition Reports allow us to reflect each year on supports, review, collaborate with all providers and plan for a successful new school year.
Our goal is to make sure that you have a strong relationship with your child's school and that they are receiving the appropriate support required under the CRPD, DDA & DSE.
SUPPORT WORKER PARTICIPANT ENGAGEMENT PLANS
We provide organisations with Engagement Plans for their support workers.

This includes:

Development of participant engagement plan for support workers to follow at visits.

Development of engagement plan that uses SMART criteria.

Allows support workers to have an understanding of the client, their needs and have achievable and measurable goals.

Engagement plans assist at reviews.

Developed by tertiary qualified Social Workers with expertise in plan development and engagement.
MENTAL HEALTH CHECK INS
Mental Health Check Ins support young people with short weekly or fortnightly counselling sessions of 15- 30 minutes. Our Social Workers monitor their mental health through establishing strong and genuine relationships. We also support the entire family unit.

PARENT & CARER SUPPORT
The hardest job in the world is parenting. However, when you have a child with a disability, parenting becomes more complex and tiring. Working 1:1 with our Social Workers, we give you strategies to support your child, building your capacity, giving you breathing space and a safe place to express your concerns and build your confidence.
Fees
Social Workers can charge the NDIS under the following line items in accordance with the NDIS pricing arrangements and price limits 2024-2025.
